(Dubuque, IA) -- An assisted living home for people with intellectual disabilities in eastern Iowa is being fined almost $14,000 for violations.
The West 32nd Community Living Home in Dubuque is facing $13,750 in fines from the Iowa Department of Inspections and Appeals.
The facility has 14 federal violations and seven state violations.
After an investigation, inspectors found the facility failed to provide care during evening hours and on weekends, assess injuries or medical conditions in a timely manner, and follow policies to prevent abuse and neglect.
