(Urbandale, IA) -- The Iowa Department of Inspections and Appeals issued two state violations against Glen Oaks Alzheimer’s Special Care Center after they falsely pronounced a resident dead last month.

A licensed practical nurse at the Urbandale care facility said the resident was dead at 6 a.m. on January 3. A funeral director then put the woman in a body bag and transported her to the Ankeny Funeral Home and Crematory.

The DIA says an employee at the funeral home discovered the woman was alive and called 911.

After medical examinations, she was brought back to the care facility. The woman died on January 5.

The DIA violations will cost Glen Oaks Alzheimer’s Special Care Center $10,000.
